  • Monthly premium is based on your income, age, and family size.

How to Apply

  • Call Basic Health at (800) 826-2444 for an application.
  • Local sites that may have applications in stock - SHIBA (Statewide health Insurance Benefit Advisors), Clark County Public Health, North and East County Family Resource Centers, and Free Clinic of Southwest Washington
