- For birth certificates from 1895 through 1907, contact the Clark County Auditor's Office in the Public Service Center, second floor, at 1300 Franklin St., Vancouver WA.
- For birth certificates from 1908 through 1923, contact the Washington State Vital Records office in Olympia at (360) 236-4300.
- For birth certificates from 1924 to present for the State of Washington, submit a request to Clark County Public Health by:
- Filling out an application at the Vital Records counter located on the third floor of the Clark County Center for Community Health, 1601 E. Fourth Plain Blvd., Vancouver between 8:30 a.m. - 4:30 p.m.
- Mailing a request to Clark County Public Health, P.O. Box 9825, Vancouver, WA 98666-8825.

- The information needed for a birth certificate requested at the Public Health office is:
- Full name at birth
- Date and place of birth
- Both parents' names, including the mother's maiden name
- Mailing address and daytime phone number
- The cost per certified birth certificate is $20 for copies ordered at either the state or local level. (Revenue from this fee is used for developing and maintaining the state vital records systems, including a Web-based electronic death registration system.)
- You may pay by cash, check, VISA and MasterCard. For other credit cards, you must order online. If paying by check, make check out to "CCHD." If a birth certificate cannot be found, you will be charged an $8.00 search fee.
- Birth certificates are processed Monday through Friday, 8:30 am - 4:30 pm. Home birth registration is by appointment only. Call (360) 397-8092 to make an appointment. Please ask for additional information regarding corrections on birth certificates.
