Chronic diseases such as heart disease, diabetes, cancer and stroke are the leading causes of disease and disability, accounting for 7 out of 10 deaths among Americans each year. These diseases also account for $3 of every $4 spent on health care.

With the help of the Community Transformation Grant from Centers for Disease Control and Prevention (CDC), Public Health and community partners are promoting and supporting healthy lifestyles in all neighborhoods, especially those experiencing the greatest burden of disease. This grant focuses on the root causes of poor health and making improvements where people, live, learn, work, play and go to school, especially for those populations most affected by disparities and health inequities.

Healthy eating

Easy access to healthy food and beverages in communities and workplaces. People eating more fruits, veggies and other healthful foods.

The local Food System Council increases and preserves access to safe, local and healthy food for all residents of Clark County. The council supports a viable, economical and sustainable local food system through multiple strategies.

Active living

More streets are safe and walkable, especially for students.

Preventive healthcare services

More technical assistance for health care providers to strengthen preventive services. Fewer people with obesity, hypertension, high cholesterol and late-stage cancer.

Tobacco-free living

Increased access to smoke-free housing, parks and campuses. Greater availability of services to help people stop smoking. Local control over tobacco promotion and taxation. More people living tobacco free.
