Apply for WIC

If you want to apply for WIC benefits, please call your closest WIC office to find out if you are eligible. You will be asked for your name, address, birth date, household income, and pregnancy status.

If you qualify, a 30-60 minute appointment will be scheduled to complete the application process and answer any questions you have. All family members eligible for WIC need to attend appointments including children.

Where are the WIC offices located?

Do I qualify for benefits?

You qualify for benefits if you:

  • Live in Clark County.
  • Are pregnant, postpartum, breastfeeding an infant under 1 year of age, or a child under five years of age.
  • Have a medical or nutritional need. (This is determined during your appointment.)
  • Meet the income guidelines (PDF).

Fathers, grandparents, foster parents and other legal guardians are encouraged to apply for services for eligible children under their care. 

What information do I need to bring?

You must provide

* During pregnancy, a medical verification of pregnancy is also required.

Identification
You must show identification for yourself and your children. Proof of identification may include:

  • Driver's license or state identification card.
  • Military ID.
  • Birth certificate.
  • Immunization record.
  • Medicaid record.
  • Hospital birth record.
  • Work or school ID.
  • Passport.
  • Voter registration card.
  • Social Security card.
  • Voter registration card.

Proof of income
Proof of income may include:

  • Pay stubs.
  • Unemployment benefits stub.
  • A document from your employer.
  • TANF notice of approval or change notice letter .
  • Medical ID card or award letter (not health plan card).
  • Food stamp award letter.

Proof of address
Address verification may include:

  • Medicaid card
  • Rent or mortgage receipts
  • State or local document bearing name and address
  • Washington ID card
  • Health insurance card with address
