Improving community health
Chronic diseases such as heart disease, diabetes, cancer and stroke are the leading causes of disease and disability, accounting for 7 out of 10 deaths among Americans each year. These diseases also account for $3 of every $4 spent on health care.
Public Health and community partners are promoting and supporting healthy lifestyles in all neighborhoods, especially those experiencing the greatest burden of disease. Our work focuses on the root causes of poor health and making improvements where people, live, learn, work, play and go to school, especially for those populations most affected by disparities and health inequities.
Strategic changes. Sustainable results.
