Vancouver, Wash. – Clark County Public Health is seeking nominations for the annual Public Health Community Award, which recognizes both a local individual and organization for outstanding contributions in promoting and protecting community health.

Nomination forms are available at www.clark.wa.gov/public-health/community-award-nomination.  Completed forms can be submitted online, by mail or in person. Details are on the form.

Nominations are due by 5 p.m. Wednesday, April 5.

“This award acknowledges the vital contributions of our community partners,” said Stephanie Roise-Yamashita, Public Health Advisory Council chair. “Because Public Health can’t do it all, these individuals and organizations are essential to addressing various health issues in Clark County.”

A committee of council members and Public Health staff will review nominations and select the winners. The committee will give special consideration to individuals or organizations in Clark County that work to prevent health problems, forge partnerships and strive for communitywide outcomes.

The award will be presented during the Clark County Board of Health meeting at 8:30 a.m. Wednesday, April 26, in the sixth-floor hearing room of the Public Service Center, 1300 Franklin St.
