How do I order a birth certificate?
- For a birth date of 1895 through 1907, contact the Clark County Auditor’s Office.
- For a birth date of 1907 to the present in Washington, submit a completed application in English (DOC) or Spanish
(DOC)
- In person - Clark County Center for Community Health, Vital Records, 3rd Floor 1601 E Fourth Plain Blvd., Vancouver. Office hours are Monday, Tuesday, Thursday and Friday: 8:00 a.m. - 4:30 p.m.; Wednesday: 9:00 a.m. - 4:30 p.m.
- By mail - Clark County Public Health, P.O. Box 9825, Vancouver, WA 98666-8825.
- By FAX - Clark County Public Health, Vital Records, (360) 397-8091.
- Online - Clark County partners with an independent company to provide online service. VitalChek can be reached through its website, www.vitalchek.com. An additional fee is charge by VitalChek for using this service. All major credit cards are accepted.
- Home birth registration is by appointment only. Call (360) 397-8092 to make an appointment.
What information do I need?
- Full name at birth.
- Date and place of birth.
- Both parents' names, including the mother's maiden name.
- Your mailing address and daytime phone number.
How much does it cost?
- Each certificate costs $20.
- If an application is processed and no record is found, there is an $8 search fee.
You may pay by cash, debit card, VISA and MasterCard. For other credit cards, you must order online. For additional information call (360) 397-8414.
How do I order a death certificate?
- For a death prior to 2002, contact Washington State Vital Records at (360) 236-4300.
- For death certificates from 2002 through present in Clark County only, submit a completed application (DOC):
